Who provides medical care

Medical care accessed through SYN Now is provided by the independent medical group responsible for your care and its physicians, nurse practitioners, or other licensed healthcare professionals (collectively, "Providers"). Providers are responsible for evaluating patients, determining whether telehealth is appropriate, making clinical decisions, issuing prescriptions when appropriate, and providing clinical follow-up.

SYN Health, Inc., doing business as SYN Now, provides non-clinical technology, marketing, customer support, and administrative services. It does not direct a Provider's clinical judgment, practice medicine, issue prescriptions, or dispense medication.

Provider identification

Before care is delivered, patients are provided information identifying the treating Provider and the professional medical entity responsible for the encounter. This information includes the Provider's name and professional credentials and, where required, state license or other public identifying information. The patient portal and clinical documents associated with your encounter should be used to identify the Provider responsible for your care.

To request a Provider's full name, credentials, NPI, licensing jurisdiction, or license number, contact support@synnow.com. Requests will be routed to the applicable medical group. Licensure can also be verified through the medical board or professional licensing agency in the state where you were located during the encounter.

Licensure and state availability

A Provider may treat a patient only when authorized to practice in the jurisdiction where the patient is physically located at the time of care. SYN Now checks location and routes patients based on provider availability, requested treatment, pharmacy capability, and applicable telehealth rules. Availability may change and completing an intake does not guarantee that a Provider is available or that treatment will be prescribed.

Pharmacy services

Prescription fulfillment and dispensing are performed by the dispensing pharmacy, an independent licensed pharmacy, or another pharmacy selected or permitted for the prescription. The pharmacy is responsible for compounding when ordered, dispensing, labeling, pharmacist counseling, and shipping in accordance with applicable law. Pharmacy identity and contact information appear on the medication label and shipment materials.

Clinical questions and complaints

Contact your Provider through the patient portal for questions about symptoms, diagnosis, medication, side effects, or follow-up. For help locating Provider information or submitting a complaint, email support@synnow.com. You may also contact the Provider's state licensing board. Nothing on this page limits your right to file a complaint with a professional licensing or regulatory authority.
