Compounded medications

Some treatments available through SYN Now are compounded for an individual patient by a licensed pharmacy after a Provider determines that a compounded preparation is clinically appropriate. Compounded medications are not FDA-approved. FDA does not review compounded products for safety, effectiveness, or quality before they are marketed, and a compounded product is not an FDA-approved generic equivalent. Before treatment

Tell your Provider about all health conditions, including:

• heart disease, chest pain, heart attack, stroke, abnormal blood pressure, or fainting;
• kidney or liver disease, bleeding disorders, blood-cell disorders, or stomach ulcers;
• eye or hearing conditions;
• penile conditions or a history of prolonged erection;
• mental-health conditions, impulse-control symptoms, or severe sleepiness;
• pregnancy, possible pregnancy, breastfeeding, or plans to become pregnant; and
• all allergies and all prescription drugs, non-prescription drugs, vitamins, and supplements.

Do not combine sexual-wellness medication with another erectile-dysfunction or arousal treatment unless the prescribing Provider specifically directs you to do so. Do not use a prescription issued for another person or share yours.

PDE5 inhibitors

Some formulations may contain sildenafil, tadalafil, or vardenafil. Do not use a product containing a PDE5 inhibitor with any nitrate medicine or recreational nitrate ("poppers") or with riociguat; the combination can cause a dangerous drop in blood pressure. Tell your Provider about alpha blockers, blood-pressure medicines, antifungals, antibiotics, HIV medicines, and other drugs that may interact.

Potential effects include headache, flushing, indigestion, nasal congestion, dizziness, back or muscle pain, and changes in vision. Serious risks can include very low blood pressure, sudden vision or hearing loss, and priapism. Sexual activity can strain the heart; ask your Provider whether sexual activity and treatment are appropriate for you.

Apomorphine-containing formulations

Apomorphine may cause nausea, vomiting, dizziness, sleepiness, fainting, or low blood pressure. Avoid alcohol and do not drive or operate machinery until you know how it affects you. Tell your Provider if you take ondansetron or another 5-HT3 antagonist, dopamine-blocking medicine, or blood-pressure medicine because serious interactions may occur. Report new compulsive urges, hallucinations, severe sleepiness, or fainting promptly.

Bremelanotide- or oxytocin-containing formulations

Bremelanotide can temporarily increase blood pressure and lower heart rate and should not be used by people with uncontrolled high blood pressure or known cardiovascular disease. Nausea, vomiting, headache, flushing, injection-site or application reactions, and darkening of skin or gums may occur depending on formulation and route.

Bremelanotide and oxytocin-containing products may present pregnancy-related risks. Oxytocin can stimulate uterine contractions. Do not use these ingredients if you are pregnant, could be pregnant, or are breastfeeding unless your Provider has specifically reviewed the risks and instructed you to use the prescribed product. Uses or dosage forms in compounded sexual-wellness preparations may not be FDA-approved.

Topical and intranasal formulations

Creams and sprays may cause burning, irritation, redness, numbness, headache, nasal symptoms, dizziness, or unintended exposure to another person. Apply only where and how the label instructs, wash hands after use, avoid eyes and unintended mucosal contact, and follow directions about skin-to-skin contact. Do not use a damaged or contaminated dispenser.

How to use medication safely

• Follow the exact dose, timing, route, storage conditions, and expiration date on the pharmacy label.
• Do not take an extra dose because the expected effect did not occur.
• Contact the Provider before restarting after a new diagnosis, medication, hospitalization, or significant health change.
• Keep medication away from children and other people and dispose of it using pharmacy or local take-back guidance.

Side effects and adverse events

Contact your Provider through the patient portal for side effects or clinical questions.
