Licensed medical providers and dispensing pharmacies are independent organizations and may maintain separate privacy notices. Personal information we collect

Information you provide

• Identity and contact information, such as your name, date of birth, address, email address, telephone number, and account credentials.
• Health and treatment information, such as medical history, symptoms, allergies, medications, photographs, questionnaire responses, treatment preferences, prescriptions, and messages with a provider.
• Transaction information, such as items ordered, subscription status, shipping details, and billing history. Payment-card data is processed by our payment processor and may not be stored by us.
• Communications, including support requests, survey responses, reviews, and other correspondence.
• Marketing preferences, including your choices about email, telephone, and text-message communications.

Information collected automatically

When you use the Services, we and service providers acting on our behalf may collect device and usage information, including IP address, browser type, device identifiers, referring page, pages viewed, approximate location derived from IP address, access time, and interactions with the Services. • support clinical intake, provider review, prescription fulfillment, shipping, refills, and follow-up care;

• maintain, secure, troubleshoot, and improve the Services;
• prevent fraud, enforce our terms, and comply with legal and regulatory obligations;
• create aggregated or de-identified information; and

We do not knowingly use clinical intake answers, prescription information, provider messages, or other PHI for interest-based advertising.

How we disclose personal information

We may disclose personal information to:

• Medical providers and pharmacies to evaluate treatment requests, provide care, issue and fill prescriptions, and coordinate follow-up.
• Service providers supporting hosting, security, identity verification, communications, payment processing, analytics, customer support, shipping, and other business functions.
• Advertising and referral partners for permitted campaign measurement and marketing. Depending on applicable law, some cookie-based disclosures may be considered "sharing," "targeted advertising," or a "sale," even when no money is exchanged.
• Professional advisers such as attorneys, auditors, accountants, and insurers.
• Authorities or other parties when reasonably necessary to comply with law, respond to valid legal process, investigate fraud, protect safety or rights, or enforce agreements.

Rights relating to medical records maintained by your provider should be directed to that provider through the patient portal or the contact information in the applicable Notice of Privacy Practices.

Security and retention

We use administrative, technical, and physical safeguards designed to protect personal information. No transmission or storage system is completely secure. We retain information for as long as reasonably necessary for the purposes described in this policy, including clinical, legal, accounting, fraud-prevention, and regulatory obligations. Medical records may be retained for periods required by the law applicable to your provider.

Children

The Services are intended for adults age 18 and older and are not directed to children. We do not knowingly collect personal information from children through the Services. If you believe a child has provided information, contact us so we can review and address it.

Third-party services

The Services may link to websites or applications we do not control, including the patient portal. Their privacy practices may differ from ours. Review the privacy notice presented by each service you use.

Changes to this policy

We may update this policy as our practices or legal obligations change. We will post the revised policy with a new "Last updated" date and provide additional notice when required.
